To begin the process, please complete the Intake Form and email it us, along with the following

- Please send a photo of your insurance card.

- Insurance subscriber details:

  - Full Name

Once we receive your information, we will create a patient account for you, and you will receive an email from noreplypatientonlineportal.com. We will schedule your appointment in the earliest available slot.  


Our intake sessions last 90 minutes and are conducted in person. Please be aware that your appointment will be canceled unless the patient portal is completed 24 hours before your scheduled time. All follow-up appointments will be arranged based on the services provided.

Insurance Information

Each insurance plan is distinct, and it's essential for you, as our valued client, to understand your insurance benefits. We highly recommend that you speak directly with your insurance provider before beginning your treatment to clarify their benefits and limitations. Please keep in mind that Behavioral Health Resiliency PLLC is not involved in the contract between you and your insurance provider, highlighting the importance of your direct communication with them.

We are currently in contract with the following insurance companies: Aetna, Anthem, Blue Cross Blue Shield, Cigna, Optum, Tufts, and United Behavioral Health.

We do not accept Medicare or Medicaid. 
We do not bill secondary insurance.  
To ensure you receive the most benefit, please reach out to your insurance provider with any questions about your coverage.  When you contact your insurance company, you may want to ask these questions to better understand your benefits: 

 

Is Behavioral Health Resiliency PLLC included as a covered provider in my plan? Is this provider in-network or out-of?

What is my deductible for services?

What are the copay or coinsurance amounts?

What will my financial responsibility be?

Will I receive reimbursement for out-of-network services with Behavioral Health Resiliency PLLC?

Investment in You

The fees when paying privately for initial new patient evaluation intake appointment is $375.00.

 

The follow up appointments which includes medication management and psychotherapy services are $200.00

Fees for Missed or Late Cancelled appointments is $125.00. â€‹â€‹

An appointment is considered a late cancellation if not cancelled at least 24 hours before the scheduled time.

As an outpatient provider, we offer non-emergency care in an outpatient setting. 

As your Psychiatric Nurse Practitioner, we will work to meet your needs during our scheduled appointments. Sometimes, life can up questions or concerns between visits.

 

Your privacy is a priority, and we have a HIPAA-compliant patient portal for established clients to reach out.

 

If concern requires further discussion, we can conveniently schedule an appointment, either through telehealth or in-person.

After you are through the intersection and on Greeley Street, you will take the FIRST LEFT

Park immediately. Unit 8B is the second Unit.  You will go up the stairs, and come in and make yourself comfortable.
